Copy the form.phtml from the path
**vendor/magento/module-contact/view/frontend/templates**

and paste it into the
**app/code/Vendor/Extension/view/frontend/templates**

**Step 1:** File Path : **app\code\Vendor\Extension\view\frontend\templates\form.phtml**
